Preoperative anaemia is a negative factor in aortic valve surgery.

چکیده
The impact of anaemia on patients undergoing aortic valve surgery is poorly understood. Anaemia is associated with aortic valve surgery. A recent article by Elmistekawy et al. [1] demonstrated that preoperative anaemia is a common finding in patients undergoing aortic valve surgery and is an important and potentially modifiable risk factor for postoperative morbidity and mortality. This analysis suggests that anaemia may serve as a negative factor in aortic valve surgery. Other studies have been carried out on this topic. Major surgery in a patient with pancytopenia might be associated with increased surgical risks, particularly for bleeding and infection. Halliday et al. [2] demonstrated that, in a logistic regression model, coexisting vascular disease, diabetes and preprocedural anaemia significantly affected the incidence of life-threatening or major bleeding. Furthermore, Kitamura et al. [3] have shown that postoperatively, a patient developed paravalvular leak and haemolytic anaemia, subsequently undergoing reoperation. Also, Van Mieghem et al. [4] found that baseline anaemia is common in patients undergoing transcatheter aortic valve implantation, forecasts a need for more red blood cell transfusions and is associated with increased 1-year mortality. Moreover, hypercoagulative states have been observed in patients with alpha-thalassaemia as beta-thalassaemia; therefore, special attention should be taken in perioperative anticoagulation therapy. Kawase et al. [5] indicated that haemolytic anaemia with aortic stenosis resolved by urgent aortic valve replacement. However, Lee and Lee [6] reported a successful cardiac surgery with extracorporeal cardiopulmonary bypass in a patient with severe aortic valve insufficiency and concomitant idiopathic aplastic anaemia. In summary, these findings suggest that anaemia may be a negative factor in aortic valve surgery. Therapeutic agents targeting anaemia might result in innovative new therapies for aortic valve surgery. Finally, we greatly enjoyed reading the article by Elmistekawy et al. and believe that the prevention and treatment of anaemia may be useful in aortic valve surgery.

Can the female gender influence risk factor identification for aortic valve surgery?
We have read the interesting article by Elmistekawy et al. [1], which showed that preoperative haemoglobin (Hb) was an independent predictor of mortality after aortic valve replacement (AVR) [1]. Although the authors defined that anaemia depends on gender and showed no difference in the incidence of preoperative anaemia between both males and females [1], we believe that gender should be consid...
متن کاملAortic Valve replacement with Reduction Aortoplasty with Mesh Prosthesis in 75 years old with Sever aortic Regurgitation with Aortic Aneurysm
Although classic and standard procedure for patients with ascending Aorta aneurysm with Sever aortic Regurgitation is Bentall surgery but in some selected cases this complex surgery was not performed with different reason like advanced age and coexisting comorbidity involving renal and hepatic and lung Function. In our cases 75 years old man with chronic renal failure ( Cr= 3.2 ) who was known ...
متن کاملReply to Tao et al.
We would like to thank Tao et al. [1] for their interest in our article [2]. As the authors of the letter point out, our study clearly demonstrates that preoperative anaemia is an important risk factor for morbidity and mortality after aortic valve surgery. While cardiac surgical procedures can be safely performed in this population, anaemic patients experience perioperative complications more ...
In recent socio-economic state it is not possible to have different sets of prosthetic cardiac valves available in the operating room before open-heart surgery for valvular replacement. In this study the diameter of the aortic valve ring measured in 2-D echocardiography was compared with the size of the aortic prosthetic valves implanted for the patients with aortic valvular disease. The purpos...
متن کاملSerratia Marcescens, An Opportunistic Gram Negative Infection In Cardiac Valve Surgery
Serratia marcescens (S.M) is a species of gram–negative bacteria in the family enterobacteriaceae. A human pathogen, S. marcescens is involved in nosocomial infections, particularly in urinary tract and wound infections. This report describes a 63 years-old man that referred to hospital with dyspnea FC IV and he underwent cardiac valves replacement surgery with a diagnosis...
